All required immunizations and health forms listed below are to be completed and submitted via the upon entrance (for ALL Residents and Traditional Undergraduate Students).

Incoming students will have access to their Student Medicat Connect portal after enrolling and depositing within 2-3 days.

For Fall admits: If required paperwork is not received by July 1st, a HS HOLD will be placed on the student's account which limits usage of university access.

For Spring admits: If required paperwork is not received by January 18th, a HS HOLD will be placed on the student's account which limits usage of university access.

Required immunizations:

  • Influenza Vaccine: annually
  • MMR Vaccine #1, #2 (Measles, Mumps, Rubella): 2 doses required or positive blood titers.
  • Meningococcal ACWY Vaccine (MCV4)
  • Meningococcal B Vaccine #1, #2: Two doses required of the Bexsero or Trumenba. Will accept Penbraya series.
  • Tdap Vaccine (Tetanus, Diphtheria, Pertussis): one dose given within the past 10 years.
  • Varicella Vaccine #1, #2 (Chickenpox): two doses required or positive blood titers if you had the disease.
  • International Students Only: QuantiFERON TB-Gold blood test or Chest X-Ray completed within the past 2 years. Results must be NEGATIVE and verified in writing by examining Physician.

Exemptions from vaccine requirements are accepted for verified medical and religious reasons.

Highly recommended immunizations:

  • Gardasil Vaccine #1, #2, #3 (HPV): series of three doses
  • Hepatitis A Vaccine #1, 2 (HAV): series of two doses
  • Hepatitis B Vaccine #1, #2, #3 (HBV): series of three doses or positive blood titers
  • COVID-19 Vaccine series

  • Health History : This document is to be completed by the student. All information is confidential. If under the age of 18, a parent/legal guardian signature is required.
  • Tuberculosis Screening : This document is to be completed by the student and is a screening tool used to determine potential exposure to active Tuberculosis. You will be contacted by one of the Registered Nurses in 国产传媒 Student Health Services if it is determined that clarification or testing is needed.
